We do have nurse visits if needed for services such as:

The Student Health Center offers various types of physicals. Some examples are:
- General Routine Physical
- Participation Physicals for entry into a program or activity

Each type of physical generally requires specific forms that are often provided by the agency or entity requesting the physical. It is the patient’s responsibility to arrive with the required form to complete the appointment; if you do not have all necessary information your appointment may be rescheduled.
Injury Follow Up
If you have sustained an injury that you received treatment for in a local urgent care or emergency facility and would like to follow up at the Student Health Center you can contact us to inquire about an appointment provided that you can:
- Bring all records from your care at another facility with you to your appointment
- The injury is not related to Worker’s Compensation
**In certain cases we may advise you that we need to review your medical records prior to scheduling follow up with us to determine if we can meet your continued needs.
