About the Texas State Law for Bacterial Meningitis

Effective January 1, 2014 Texas state law ( Senate Bill 62) requires that students who will be under the age of 22 on their first day of class at an institution of higher education in Texas, 麻豆精品, must provide proof of immunization OR an accepted waiver for bacterial meningitis. The vaccination must have been received during the 5 years prior to enrollment..

The Texas Higher Education Coordinating Board specifically requires one of two specific classes of meningitis vaccines:
Meningococcal conjugate vaccine (MenACWY). Brand names, Menveo and Menactra. Meningococcal polysaccharide vaccine (MPSV). Brand name, Menomune.

Meningococcal serogroup B (MenB) vaccines do not provide the necessary coverage for college students and are not accepted for admission to higher education. The specific brands not accepted are Trumenba and BexSero.

How To Submit Your Meningitis Vaccination

If you are under the age of 22 on your first day of class:

SHSU will require you to either your evidence of vaccine or one of the approved methods of waiver from the vaccine before being able to register for classes or secure on campus housing if you are:

  • A returning student who did NOT attend classes at SHSU the preceding semester
  • You are an entering first time freshman
  • You are an entering first time transfers student
  • You are an entering graduate or post-baccalaureate student
  • You are a "hybrid" student (taking classes both on campus and online)

Please complete submission through the Patient Portal with one of the following processes:

To submit your shot record online:

  • Log onto the or by accessing it through the Student Health Center website
  • Your student username and password will be your log on
  • Click on the Immunizations tab on the top menu bar
  • Enter the date you received your immunization
  • Click the upload tab and upload a copy of your immunization record

**You must do BOTH steps for it to be accepted**

To submit our evidence of vaccination form signed by your Physician:

  • Download the form from our website
  • Complete the top section of the form
  • Your Physician must complete and sign the bottom section
  • Log onto the or by accessing it through the Student Health Center website
  • Your student username and password will be your log on
  • Click on the Immunizations tab on the top menu bar
  • Enter the date you received your immunization
  • Click the upload tab and upload/submit the form.

**You must do BOTH steps for it to be accepted**


Exemptions

You are exempt if you are 22 years of age or older as of January 1 of the year you are enrolling in higher education.

To claim a medical waiver exemption:

  • A medical affidavit or certificate signed by a physician registered and licensed to practice medicine in the United States stating that in the opinion of the physician the required meningitis vaccination would be injurious to the health and well-being of the student
  • This affidavit must have the contact information for the physician writing it provided
  • This letter will only be good for 1 year and new documentation will be required upon its expiration; unless your physician indicates you have a lifelong condition
  • Follow the steps to upload the letter to your Patient Portal for review

To request a waiver of conscience:

  • This must be done through the State of Texas Department of State Health Services
  • Allow 3-4 weeks for completion of this process
  • You must request the "Exemption from Immunizations Reasons of Conscience Affidavit Form" from the Texas Department of State Health Services. You may find this form at
